It's when getting something done in (or by) an institution that doesn't 'fit' with that organization's culture seems to just not happen. It's not malice, and it isn't even incompetence. It's more like "I've got this stack of items to do, and I'll do the ones I like first". Your item, unliked and/or 'not invented here' will keep getting shuffled to tomorrow.
